Role Overview
We are a U.S.-registered digital health startup preparing a CMS ACCESS Model reporting MVP. We are looking for a Senior FHIR R4 / Healthcare Interoperability Contractor to help design and implement a de-identified clinical data pipeline, FHIR mapping, API workflow, audit trail, and data provenance structure.
Phase 1 will use simulated or de-identified data only. No live PHI access is required.
Responsibilities
Design and implement a de-identified clinical data pipeline aligned with healthcare interoperability standards.
Develop FHIR R4 mapping and API workflows to support CMS-style reporting readiness, submission tracking, and status monitoring.
Create an audit trail linking source documents, structured fields, review status, and reporting outputs.
Advise on HIPAA-aware architecture, including access control, audit logging, data minimization, and de-identified data workflows.
Collaborate with clinical and operations stakeholders to define data fields, validation rules, and CMS/Medicare value-based care reporting workflows.
Required Skills
Expertise in HL7/FHIR R4 standards and healthcare APIs.
Experience with HAPI FHIR, Mirth Connect, Orion Rhapsody, or similar healthcare interoperability tools.
Ability to design clinical data pipelines, FHIR mapping, API workflows, audit trails, and data provenance structures.
Understanding of HIPAA-aware healthcare architecture, including access control, audit logging, data minimization, and de-identified data workflows.
Experience with CMS, Medicare, value-based care reporting, payer reporting, or quality reporting is strongly preferred.
Nice to Have
Familiarity with SMART on FHIR applications.
Experience working with FHIR Implementation Guides or evolving draft specifications.
Ability to work effectively in a remote setting with some overlap in EST.
